Current State: I have a very enhanced epic issue type with many custom fields that we are looking to capture detail. This issue type does have its own screen layout already.
Problem statement: I need to create a screen/screen scheme for when the issue is created to limit the amount of fields that are displayed.
Here is what I currently attempted that is not working.
Needless to say this is not providing the expected results and am hoping this amazing community can assist with where I missed an action.

I can't see Image #1, #2, and #3 (maybe I'm too early) but in your screen scheme, is the new screen that you created in Step 1 assigned to only the "Create Issue" operation of the screen scheme that you created in Step 2?
Here's an example of a screen scheme that has a different screen for the create operation and the view/edit operation. You'll want something set up like this to accomplish your objective.

Hmmm from your screenshots it looks like you have the right setup.
Could you include a screenshot of the MSCDS: Epic Issue Screen please?
And as a clarifying question, can you provide specifics as to what you are seeing when you create an Epic in the create screen and how they differ from your expected results? Don't want to make any more assumptions if I can help it.

Here is the screen shot you requested, This also corresponds with the fields and order that I see (Admin/Instance owner) when I create an new issue.
However, my leader has a different view seen below when she opens them. She can click on the ellipses and add fields but I don't know why they only show a few.

Hmm does she have the option to view all fields in that ellipses menu? Is it currently selected to "Custom Fields"
So it sounds like this current configuration you have is giving you the expected results, but is not giving your leader the expected results?

@Sam Nadarajan I am happy to report your screen shots helped me a lot. I was able to determine that the screen I had for "Create view" was associated in another screen scheme that showed all the fields. Once I removed that association it rectified the issue and will be aware of that the next time this comes up. Thank you for your assistance.
