You need to make sure the tool is enabled, however I will caution that the only option that worked for me made it show up for EVERYONE.
Under Manage Apps, you'll need to click on the Configure button in the app details for Entity Property Tool. You should see this screen:
Unfortunately selecting the second checkbox (Personally enabled) did not work for me. The option never "stuck". And if you check Global, then regular users will see Entity Properties for Issues, which may confuse or alarm them:
HOWEVER, it turns out that if you leave Global unchecked, but the add-on is still installed, you can still access things like Entity Properties for Projects via a URL like:
And from there, you can view/update/add properties.
Also, checking "Enable for me" on the screen that this shows up did not work:
It would be nice if the tool got a little love. As @Glenn Bullingham pointed out in his review, there's other issues, like the inability to (or view?) User Properties for other users.
