I have a user Jira Internal Directory account create and automatically assigned to Service desk and jira by Author JIRA. I have no idea how this would happen.

Just to make you aware, you've included a lot of personal information in your screenshot. I'd recommend against that in the future.
As for the question, I'm not seeing where it assigned the user to JIRA Service Desk/JIRA. There are several configurations that can be set up within JIRA to create users and add them to groups automatically when they log in.
Is this user a service desk customer? Or you mean they were given agent permissions to Service Desk?

I was able to replicate this with public mode enabled and my application defaults set up to Service Desk, and Software.
Can you check an see what your application settings are? Administration>Applications>Application access and then click on "Set defaults for new users" located in the upper right hand corner.
Likely what is happening is that you've got JIRA set up so that when new users are created they are added to those groups. If public mode is turned on, JIRA will allow users to sign themselves up and it will be reflected exactly like your screenshot.
To check public/private mode, go to Administration>System>General Configuration (Or just type the period key and type in "General Configuraiton"). Once there, you should see that the second line in the table is the "JIRA" mode. If it is public, this may answer your question.
