I created an advanced form but it is not showing up on the forms tab. What step am I missing?

We might need a little bit more information (and maybe some screenshots) on the steps you have performed thus far. In which type of project are you looking to us Forms?
If you are working with forms in a Request type in Jira Service management you need to find and link the form to the request type via the "attached form"-field on the request type configuration.

Sure! I'm working in JWM (Jira Work Management) and selected 'Advanced Forms' from the Project Settings option. I selected Issue type = Task. I created my form using a Template and also added a few custom fields. The form on Preview looks great! It just does not show up as a form on the Project Forms tab. There seems to be no way to "publish" the form. Please advise.

I suspect you are perhaps still using the Proforma addon for this. Atlassian bought the proforma addon and then incorporated it into the Jira product and you seem to have a "bit of both". It would explain that the forms you create in advanced forms is not popping up in the JWM tab for forms.
Just to confirm, you are on jira cloud premium? Not server/ data center? or have you recently migrated from server/datacenter to cloud?

I would recommend you to build your form via the form tab on the project (ie the now native form feature) instead of using the advanced form but maybe the documentation might offer you some pointers if you still can use the old addon.
