My project admin discovered an interesting "bug/feature" and I'm not sure if it's intended or not.
Basically, when a project admin is trying to rename a transition, that has a transition screen set (by me as a Jira Admin) after saving it, transition screen gets removed. I couldn't find any official information about it.

What type of project are you using? Company-managed? Team-managed? Business/JWM? I am not able to reproduce the problem with a workflow used by Company-managed projects.

Is Jira Cloud or Jira Server/DataCenter being used?
You mention v9.5.1, which is a Jira Server/DC version.

I'm a bit confused whether your environment is Data Center or Cloud. You give the answer DC to my question, but the answer Company Managed project to @John Funk 's question. But (as far as I know) Company Managed project only exists as a type on Jira Cloud. Cloud is also mentioned in the tags and deployment type.
I advise you to ensure that the correct environment type is stated in your question and the tags and deployment type, that way you will get the best answers.
Regarding your situation, I have tried to recreate the situation on a Server environment. When I rename the transition, the popup screen of the transition is then shown without mentioning the transition screen.
But if I publish the change immediately, the transition screen appears to be properly linked.
Are there adjustments be made to conditions or validators after changing the name?

As I've read from Is my project company-managed or team-managed? | Jira Service Management Cloud | Atlassian Support.
Project doesn't have any information about it in the side-bar, therefore it's a company managed project.
In Jira settings I checked and licence type is: Commercial (Data Center).

You indicate "Commercial (Data Center)" .
This means that you work with the Data Center variant and not in the Atlassian Cloud.
The documentation (cloud) you refer to is therefore not applicable to you. The applicable documentation can be found at jira-service-management-server .
Back to the situation you are dealing with.
Have you been able to check whether there are there adjustments be made to conditions or validators after changing the name? As I mentioned in my comment on June 22.

No conditions, validators, Post functions are changed after publishing.
Interesting part is that I don't encounter such behaviour myself because I'm Jira Admin.
Only project admin user has this problem. Screen is not there after the renaming.
