Resolution does not appear on a specific transition screen

I have many screens schemes for many issue types. One of those schemes is for a sub-task use use for peer reviews. That subtask can transition to a final status of Accepted or Rejected. The transition screen is in the Done category, and I can set the Resolution on the transition, but I cannot get the Resolution field to display on the transition screen.

These are the steps I would work through to try to find the problem:

Can you show us the Workflow Scheme for the project that shows the workflows mapped to the different issue types?

Can you then show us the workflow specifically for the issue type where you are having the problem getting the field to show?

Can you then show us each of the transitions to the statuses you are targeting, showing us the association of that transition to a screen?

Can you then show us each of those screens to confirm the presence of the Resolution field on the screen?

And lastly can you show us the actual screen presentation where you are attempting to transition the issue to the new status, where the Resolution field is missing from the screen?

Trudy, glad do. These are the things I've checked myself and why I'm stumped.
