I currently have the board grouping by issues, but some issues/tasks, despite having subtasks, are not being represented as a group and rather showing up in the 'Everything else' group.
I can't discern any difference between what is being grouped by issue and what is in 'Everything else'.
What am I missing here?

The component label on the subtasks was missing meaning they were being filtered out to their parent stories weren't promoted to the kanban swimlane as the requirement is that there are subtasks under for them to do that.

Not sure what could cause this problem. But if you don't manage to figure this out, you could contact Atlassian Support by creating a support ticket.
Also, if it happens to have our Great Gadgets app installed, you could try its Team Wallboard gadget, which can also display the contents of an the agile board by displaying swim-lanes by Stories (parent tasks).

Do you have any quick filters activated in the board?
Have you reviewed the Filter Query being used to populate the board? Is there something in it that might be excluding the sub-tasks under TP-997?

To view the Filter Query for the board click on the General tab show on the left on that same screen.
To show if you have any quick filters active we need to see a screen image of your board. Above the columns will be several pull down fields. That is the area we need to see.
