Stories show "Done" but Sprint report lists in "Removed from Sprint"

Hello!  I am trying to troubleshoot some issues with a scrum team that I have not worked with before.  I am seeing that issues are completed ahead of the sprint closure (Status = Done, Resolution = Fixed/Done, Resolved date is present, etc...) 

The Sprint report, however doesn't show any completed issues.  Instead, all of them show that they have been removed from the sprint.  The burndown for the same sprint look like a table top.  Any ideas?

Thanks for responding, Nic!  I really appreciate your help in troubleshooting!  Here is a screen shot of the board, as well as a screen shot of the workflow behind it.

Is the "done" status in the UAT column then?

That's it!  I had to go back and clarify with the person running that board, but it is already looking better.  Thanks!

For an issue to be "done", it needs to be in the far right hand column on the scrum board.  Status and resolution don't matter to scrum.

Thanks Nic.  The issues are in the rightmost column of the scrum board. Anything else I might be missing?

I'm a bit stuck.  Maybe a screenshot of the board and report might show us something?
