Here's what I've been encountering:
Couldn't submit a bug report as it continued to claim my product URL was invalid
Normally when a new issue is created there will be a brief pop-up that shows you the issue key for the new task. Are you getting that?
Are you working with a Team Managed or a Company Managed project? It will say at the bottom of the navigation pane on the left.
"On Deck" may not be the actual first status for a new issue created in the project. Can you...?
1. Show us a screen image of your board.
2. Show us the Board Settings (if this is for a Company Managed project) that shows the filter for the board and the columns/status mappings
3. Show us the workflow for the issue type you are creating.
Some of that information may be available only for administrators.

It sounds like you might have some type of browser problem going on. Does this happen on other projects? Or just this one?
Maybe try to do the same thing with another browser.

So what status is KTD-4502 in? You can't create a card "into a status". You only create a card against the project. Based on the workflow, it will put the card into the first status in the workflow. That's probably where you will find your card.
Do a search in the board search box for 4502.

Roger - it was creating the issues, but not assigning them to me automatically.
I was able to find them by removing the filter for tasks assigned to me.
Normally when creating issues on my board, it assigns them to me automatically.
Any insight as to why that would have stopped?

Actually, you sort of can, in Team Managed projects, according to my test.
In a very simple TMP with no rules assigned to the transitions, I was able to click in the In Progress column on my board and create a new issue. The system automatically created it in the To Do status and then automatically pushed it to the In Progress status.

Previously when I was viewing my board filtered by my view, and I created a new task, it would assign by default to me.
My teammates were experiencing the same thing previously, but not today
