I have a few users that stopped getting notification for new comments added. They still getting notifications for issues created. I tested the notification helper and permission scheme - no changes their. It seems that all is working as before.
Any ideas why is it happening?

If no changes were made (I'd confirm using your audit log) this likely isn't a configuration issue. Try raising a ticket with Atlassian support as they have access to the log files. They'll be able to tell you if there are errors preventing certain emails from being sent.
