I have added a On Hold step / status in my epic workflow and have added 3 transitions from different statuses to the On Hold step, but I cannot add a transition from the On Hold status back to any of the other statuses.
Can you provide a screenshot of you workflow?
And as @John Funk mentioned, what prevents you to create these transitions?

You should just be able to create transitions from the on hold status to others.

Okay, you will need to copy the workflow. Then make the changes on the new workflow while it is still inactive. Then you can add the new workflow to the workflow scheme currently being used the by the project. Match it up with the same issue types the current workflow uses. You are in effect, switching out the workflows.
Or you can try to switch over to the new workflow editor when you got in to edit the workflow. Make the changes and publish.

Why does the epic workflow behave differently to the others? On story, bug and task workflows I have edited them in exactly the same manner as I have tried here.
