I created a Form for 'Tasks' and can send the form by sharing it with a team member, the team member fills out the form and clicks submit but then nothing happens, no card is created, zero notification that a form was submitted. What happens after the submit button is clicked at the bottom of Form?
I have submitted a ticket for this issue since i do not believe the data is flowing as it is intended to.

Do you notice any error message when you submit the form ?. Also please check the HAR logs when you submit. Thanks

problem is, I am not receiving notification a card was generated and also a card is not generated. I have a ticket and will be discussing with one of your representatives today at 11.

What type of project is the form for? Get the project type information from the View All Projects page under the Projects menu.
Is the user licensed to access Jira? Can the user login to Jira and create the issue without using the form?

The form is designated for one of the 3 project categories, the user is licensed to access JIRA and create cards without the form. I also shared the form with myself, received notification via email, filled out the form, clicked submit and <<crickets>>.

This is not quite the answer I was seeking.
I am assuming you are using the native Forms capabilities of Jira Cloud. If that is not the case, please let us know what tool/app you are using to create Forms.
Forms are created by going to the Project Settings of a specific project. Any given form is used to create issues in just one project.
The answer I seek is to know the Type for the Project where the specified form is intended to create an issue.
That information is available on the View All Projects page under the Projects menu. Find the project in the list and share with use the information available for it in the Type column.
