First you need to have your custom fields created and added to the relevant ticket screen configuration.
then you need to go to the relevant workflow of tickets that are going to be created then select first Transition on Create that will take you to the first Status and add Validator, from there you will have the option to validate if those custom fields are empty a prompt will show to the user asking to fill in those fields then ticket can be created.
however, adding a custom button with + sign, i am afraid this is not a built in feature. you will just have the Create button.

This is where i want to add a button. Like right next to Summary or Description. How to do that ?

Options that you have are the following:
1- have the custom fields added by default to the create screen and will show always, you can choose to make them optional or mandatory. and can control in which location in the screen they will show (right below summary)
2- create another tab in the create screen and add those fields there.
however, adding a button in this screen and by clicking on it it will expand and show those fields i am afraid this is not doable.
