Some of the issues in our project are appearing with "place holder" in the summary field.
Those entries were not voluntarily created as such, any idea what could be the cause for their appearance?

Is there any kind of pattern in those issues? same project or issuetype? same creation time? happens to every issue? are they created by an external app?
My first theory would be someone has set up some automation or script to create issues based upon some trigger or schedule.

Thanks Jack for the suggestion, I am not aware of any such setup in our system.

Who is listed as the reporter? Is it a user or some system user? Start research there and then expand to other breadcrumbs the issues might yield.
