we have "Defect Origin" field to classify the source of the defect as QA, DEV, TAC, or PLM.
When "Defect Origin" is set to TAC while creating ticket, the following additional TAC-specific fields will be displayed and required:
Hardware Dependency: Yes / No
Topology Requirement: ≤6 nodes / >6 nodes
Configuration Complexity: CLI / Netconf / Minimal / Medium / Complex
Traffic Dependency: Yes / No
Investigation Summary Report Attached: Yes / No
Existing Workarounds: [Free text]
Frequency of Occurrence: Intermittent / Consistent
please suggest way to do this.

In the project settings under features is the forms option enabled, if so then exit the project settings and on the project side bar the Forms option should be available.
