Report in case issue transitions more than once in a status and respective assignee.

How can I create a filter or report where an issue has transitioned into a particular status more than once. Also if an issue has been transitioned more than once in a particular status, then who were the assignee.

For e.g. I have a maker checker function in my workflow. I need to know how many an issue we resend by checker to maker and how many times. Also if I can track the assignee of the issue and reason for sending back from checker to maker (custom field will track the reason from the validations).

This is basically to understand the knowledge and effectiveness of maker.

Please take a look at eazyBI report creation plugin for JIRA which allows you to create custom reports based on JIRA data.

See example report of Issues that Transitioned to Reopened status more than once. With Drag and drop you can also add other dimensions to this report or click on any cell to drill across some other dimension. For example drill across Transition Author to see who reopened the issue or Assignee to see who is the current assignee of the issue.
