I'm looking to create a form for non-Jira users to submit requests and bugs to our product and dev team. Ideally this would live in slack, so a support or onboarding team member may open the form in slack, submit, and then I would be able to delegate to the appropriate dev team in Jira.
Currently, we do not have access to Jira Service Management, so wondering if there are any alternative suggestions for accomplishing this?

There is a Slack integration for Jira Cloud that allows users to create issues directly from Slack. You can check it out here.
Also, Jira has a feature called Issue Collectors, which provides you with a form to create issues like in JSM. This form can be embedded basically everywhere. You can find more information here.
