What differences am I likely to care about if I use self-hosted versus cloud for JIRA, especially with regard to customization.
I've been a JIRA and Confluence user/administrator for many years. I started with self-hosted and have become very comfortable with customization of fields, screens, state transitions (workflow), notifications, hooks, etc.
With each client, I typically start a new JIRA instance from scratch and customize to their needs. I am not talking about moving an existing JIRA database, but am deciding whether to start my next client with a self-hosted or cloud JIRA version.
I also care about Confluence and BitBucket, but I've typically done far less customization of those.
It actually depends on the level of customization and the marketplace addons/ applications you use.
If you use a lot of addons/ applications, make sure that they are available for Jira Cloud (For Jira Server, there are approximately 1,386 addons available on marketplace whereas for Jira Cloud, there are approx 669 addons available).
Regarding customization of fields, if you are referring to creation of custom fields, you should not face any issues on this front.
You can also create custom workflows and add/ modify screen schemes in Jira Cloud.

Thank you. It sounds like the type of customizations I make are supported in both the Cloud and Server versions.
