After setting up the application link between Stash and JIRA, we do not see the stash commits in the source tab in JIRA tickets. We also have fisheye, and these do show up.
I worked around my issue by disabling trusted applications for Stash accessing JIRA, and use OAuth instead.
I am still using Trusted Applications for JIRA access Stash
I had a recent support case for this, and it turns out the issue was that one server was using NTPd and one was not. Because of this, the apparent time on the servers was more than one minute apart. Removing NTPd from the offending server and running ntpdate <server> on both machines, then deleting and recasting the AppLink, got it working again.

You shouldn't need to create entity links (project->project application links) to get Stash integration to work. If you haven't already, I'd confirm that the user you've logged in as is definitely in the "developers" group and that the permission scheme you're looking at is applied to your project. Otherwise your setup sounds perfect.

i have STASH v2.0.2 installed on amazonAWS and JIRA ondemand 5.2, all versions up to date as 14/gen/2013
in order to integrate JIRA and STASH, i've followed http://quickstart.atlassian.com/download/stash/jira-integration and now i have both side linked with OAuth
the FishEye plugin is installed and enabled in JIRA, but i can't configure it (it keeps asking for an application link, i see in jira the stash application link, but the FishEye plugin seems not aware of it existence)
my permission scheme allows "developers" to see Source Control Tab

By the way, i can't see the Source control tab!
please not that i can't share my user base between STASH and JIRA ondemand as STASH doesn't provide such functionalities.
also, I tried going project by project and adding an application link from JIRA to STASH. This can be performed by going to JIRA -> project -> administer project -> settings (left column on the bottom of the page) -> application list -> Configure -> Add Link -> choose STASH
after that, a popup throws up, asking for the corresponding STASH project key
well, this seems to be useless too :(

Ah, just saw your post over at https://answers.atlassian.com/questions/124866/can-t-link-jira-ondemand-with-stash/125369 - you've run into a known bug (https://jira.atlassian.com/browse/JRA-31330) that should be fixed shortly. Please track the issue for updates, thanks for your report!

One problem I was having in setting up the Application link was:
Authentication failed for application link Stash Certificate too old.
I fixed this by setting up ntp on both servers. I now get this error:
Duplicate i18n entry for key 'applinks.auth.provider.com.atlassian.applinks.api.auth.types.OAuthAuthenticationProvider'
