When attempting to uninstall Versatile Mermaid Renderer for Jira, the following error is displayed:
Additionally when editing a story in the roadmap, duplicates of versatile mermaid button are displayed as follows:
If I click on Allow access button, then the result is:

I work for Purple Rhapsody Tech (provider of this plugin) and I don't know if it would help in diagnosing this issue but this plugin is a completely self-contained Forge plugin - i.e. the installation and uninstallation actions should be completely handled within the Atlassian ecosystem with no callouts to the Purple Rhapsody Tech servers required to install or uninstall.
Definitely one to be reported to Atlassian because there is nothing that Purple Rhapsody Tech can do on our side to correct the multiple entries :-(
Reading @Nic Brough -Adaptavist- 's reply it might be a bug in the Atlassian installation for Forge apps that if you haven't granted access to the app before you uninstall it then it gets confused? Could you try allowing access and then seeing what happens.
Apologies for not having any better options to try - we've tried to keep this plugin simple with all data kept in the Atlassian ecosystem (including the diagram data) except for when you want to render Mermaid diagrams from external URLs.

An "unexpected error" message on Cloud needs to be reported to Atlassian, as they'll need to look at the logs and processes running on your instance in order to work out what is happening.
Assuming that you're only trying to uninstall in order to try a re-install to try to fix the problem with multiple access request boxes, and you actually want to carry on using Mermaid, then we should look at that problem, rather than the failed uninstall. What happens when you click "allow access"?
