Looking to upgrade from Jira Service Desk 3.6.1 to 3.12.0, and also understand that this will also require upgrade from Jira Core 7.4.1 to 7.9.0.
Has anyone got any real world experience in performing this, upgrade, especially around any gotcha's etc, and what specifically needs to be backed up at the file structure level, and what needs to be either overwritten or updated after the install ins complete.

Yes, there does seem to be some differing directions on how to upgrade our server environment in regards to this.

Ok here is documentation on best practices that I'm sure thousands of people have used before.
From there you can use the links on the right side of the page to get your pre upgrade instructions, execution, and post-upgrade.
I would say the most important is to test everything before doing it in production, but that will be reiterated throughout the documentation.

Thanks Patrick. Upgrade was a success; with required changes to the dbconfg.xml due to microsoft changes in sql connection.
Restored server.xml as we had our tcp port set to 8443, but we also discovered that we needed to restore out jks file (keystore) as we are using saml for user authentication.
Thanks once again for providing the document, extremely beneficial especially in backing up first, and understanding other config files that make service desk work.
