When we try to click create, we get the following message: Error occurred: unable to retrieve data from server. See logs for details. Then the screen freezes. We recently updated our add-ons, and a few weeks ago we increased our user count tier. Here is a screen shot.

As noted in the On screen error you are going to want to take a look at the logs to get a better understanding of the failure point.
I would recommend looking at the atlassian-jira.log and the atlassian-servicedesk.log files while triggering the error for the full stack trace triggered.
