When a field value is added, can I make a screen show up to add additional fields?

I work with Salesforce developers.  Right now, if someone needs to do a data load, for any reason, it just happens. No discussion, no tracking or insight into it.  We are trying to centralize this process and have a Data Load project in Jira so we have visibility into who is doing data loads, when and why.

To solve this, I've added a field out our scrum work items (Jira software project, company managed) called "data load needed".  The only options are blank or Yes. If that field is changed to Yes, then I would like a screen to appear where additional fields can be filled out and it will create a work item in our Jira Data Load project (Jira kanban, company managed).

My question is, do I have to do this on a status transition (ie - new to building), or can I trigger it whenever the field is switched to Yes?  We already have a different screen associated with the transition to close, so I don't think I can associate this additional screen there.

Can I create an automation where if the field = Yes it will bring up additional fields to create a work item?  Or can this only happen on status transition?

Unfortunately, Jira doesn't provide an opportunity to show a different screen based on field selection in transition. So, you can't open other screen from another screen.

You can use the Dynamic Forms plugin from Deviniti, which provide the opportunity to open other fields based on one field selection in one screen. You can read more in the documentation.

@Gor Greyan so if I set my custom field to Yes, then it will display the other fields I need filled out?  Will they display in their own screen? Or just on the issue where I set the field to Yes?  (either is probably fine, just trying to wrap my head around how this works).  I was envisioning a pop up screen, but it wouldn't HAVE to be a separate screen.

Would this work in a service management setting as well?  Where, when creating a ticket, a business user could select what type of issue they're having and then we'd show the fields we need them to fill out for that particular issue type?

Thanks for the follow-up question.

- If you use the add-on Dynamic Forms from Devinit, yes, once the "Data Load Needed"
 set to "Yes", the additional fields you have configured will appear on the same screen.
- Yes, it is actually a very common add-on for Jira Service Management, where it works fine.
