When creating a new issue I am filling in the Summary field and when I hit CREATE, it is erroring

When I hit the CREATE button, it is clearing out the Summary field and then throwing an error saying this field is required!  Has anyone seen this before and if so, HELP!

Can you show a screenshot of the fields that are displayed when creating an issue and the error you are seeing?  There could be a required field that is not mapped to the screen.
