When setting up a new project, how do i set it up to add fields when an option is selected

Example:

Do you like Cars - Yes or No are the options - If yes is selected it would bring up 5 more free form text boxes to allow the user to add more details/answer more questions. If no is selected you would move onto the next in the pre-defined list.

You can use Scriptrunner Behavior in JIRA for this, if you are using script runner.
