If you're open to Marketplace add-ons, Dynamic Forms for Jira  (developed by the company I work for) allows you to create smart, interactive forms with conditional fields (for example, you can show extra fields based on request type or other inputs). Here's how to get started:

1. Open the Smart Forms builder 

2. Drag and drop the elements you need (like Single-Line Text, Multi-Choice, etc.)

3. Configure each field (set labels, make it required, or add default values)

4. Use the Create New Issue or Update Existing Issue feature to map form fields to Jira fields

5. Share the form internally, externally, or embed it into Confluence or websites
