      Regarding to the upgrade, this is the main source you can use for upgrading your instance:

      To check the add-ons availability you can go to the https://marketplace.atlassian.com/ to see if they will be available in the newer JIRA version.

Check that whatever plugings you are using are compatible with JIRA 7.

Yes, you can go this upgrade with one go. However I'd advice you to check the release notes from each version you're going through and of course, run this operation in a test instance first.
