These questions are generally asked in chronological order during the project. I would like to provide reporting for my PMO, it could be done using a board, that shows the answers to these decisions per project. My thought would be to report on the decision results for all these decisions per project.

I would just use an issue filter that to pull all of the items together. If you enter them into the project in the order that you would like to review them in then you could order them on the filter by issue id. I would then place the final decision in a custom field or in the description that could be shown on the filter. This way your PMO could open the filter and see all of the information or you could schedule it to be emailed to the PMO. You could also place this information on a dashboard.
I don't know if you use Confluence but you could make reports there using the same issue filters. Hope this helps.
