I can't think of a solution to this without having to create many different sub-task issue types, but perhaps someone has a different idea without using any paid plugins -
My use case:
I'm creating a set of sub-tasks using the same sub-task issue type with different summary titles (using JMWE post-function for this) where each one (14 total) will serve a different purpose.
All the 14 sub-tasks will be split across different team members and each one of them should have a single individual question with checkbox options.
Is there a way to use the same sub-task issue type for all 14 sub-tasks without having to include all 14 individual questions in there (I don't want all team members seeing questions that don't belong to them)?
It would be great if Jira had a "hide" field property that would allow you to set visible fields on the fly, but that is not the case.
I didn't want to create 14 different sub-task issue types just for this...
I also thought about using the free checklist plugin, but I still don't know if it would allow me to programmatically say which checklist to load for each of the 14 sub-tasks.

You can do this by creating 14 different post functions using JMWE, you could also use automation for this and use the create sub-tasks action.
Here you can create multiple sub-tasks and specify the fields for each sub-task.

I was able to create all fourteen sub-tasks, each one with its own name using JMWE. but the problem is that I can't define which fields will show / hide given that the sub-task issue type's Screens and Layout must be pre-defined within the project settings.
I'm not sure what you meant by "you can create multiple sub-tasks and specify the fields for each sub-task".
My understanding is that I can't present different screens on the same issue type in Jira given those are preset in the background.
Or am I missing something here?

No that is not possible indeed, there is no option to have different screens for a single issue type in one project
Within a project you are bound the the screens defined in the screen schemes set in the issue type screen scheme.
