Created issue screen for issue type: TASK.

Looks good.

Now: we need to have this change visible when creating issue. Assigned screen to screen schema > than to Issue screen type. Seems ok; 

But my changes are not visible

I think a bit more information is needed. 

The screen is associated with a screen scheme, that is associated with an issue type screen scheme, and that is associated with your project, and you have created an issue in that project using an issue type that corresponds to all of that?

Also you might want to re-title this question.  Just a suggestion.
