Select List (multiple choices) not rendering properly

We have a select list (multiple choices) custom field called Client and its not rendering properly. Instead of showing a single dropdown, its showing multiple dropdowns. See the attachment for better understanding

What does you Velocity template looks like? What type of data are you sending into it inside the getVelocityParameters method?

We are currently having- Atlassian Template Renderer Velocity 1.6 Plugin. Field Client is a select list (multiple choice) custom field and hence no need for any getVelocityParameters method.
