Send notification to another Application while transition moved from one status to another status.

Hi There,

Is it any possible ways to send a notification to another application while transition moved from one status to another status, using any scripted post functions or any other way?

Please suggest me how to achieve this issue.

Note : We had only target post url which send notification to this and there no two way conection in b/w jira and another application.
