Tabs Not Showing on Issue Creation

I have tried everything I can find but to no avail.  I am attempting to use the Xray Default screens because they should work just fine.  When I look at the default screens, they look the way they should with the information tabs for people to fill out , however when I go to the issue creation screen, the tabs do not show.  What am I missing?, however when I go to the issue creation screen, the tabs do not show.  What am I missing

Could you recheck whether the correct screen is mapped to the create issue operation for issue type TEST in project eScribe Xray?

The screen configuration (even for general) do not match the view you show on the create operation.

Thank you for the response.  I have checked the screens and they matched, but the tabs still dont show.  The "Fields" screen shows the fields I am looking for assigned to the correct tabs, but they dont show on the Create/Issue screen.  I even went so far as to create new screens to map and they still don't show on the new screens either.  I'm beginning to believe that this plugin doesnt work with my JIRA version as my extended work with that project has crashed the system multiple times.
