Test Phase Error when attempting to create individual Test Script or a Test Plan

Wondering if anyone can offer some assistance.  I am receiving a 'Test Phase Error' when attempting to create ain individual Test Script or a Test Plan within a Project.  The error does not occur when the import process is used.  

Error is:  'Test Phase:  Test Phase is required.'

Wondering if anyone could tell me where to look to rectify this issue?
