Uninstalled app continues to run

From the PlugIn Manager Audit Log:

• Installed app Jira - Azure Integration (io.getint.apps.jira.jira-azure version 1.1.9-AC) Jul 22 2022 7:12:54 AM
• Uninstalled app Jira - Azure Integration (io.getint.apps.jira.jira-azure version 1.1.9-AC) Jul 22 2022 8:27:10 AM

In between the two I first did a "Disable" also (could not uninstall unless was first disabled).

BUT !this app continues to run  !!!

It is causing us much problems and messing our data yet we have no way to do anything, this app no longer shows up in our Jira subscription. 

CAN SOMEONE PLEASE STOP IT?
And remove it from our subscription?

as soon as possible?  thx in advance.
