Update Jira Cloud Migration instructions to clarify DB Restore zip file structure

The "Migrate from Jira Server to Jira Cloud" instructions are super confusing!  I couldn't figure out what it was saying.

I found a little ASCII diagram that says exactly how to structure the zip file.

I did a lot of searching before I found that diagram (in an answer to someone's question).  Better to have it directly in the original instructions!

  At the bottom of the page, https://confluence.atlassian.com/jirakb/migrating-from-jira-server-to-jira-cloud-779160826.html?_ga=2.97869343.1490493315.1556202877-1785046573.1549295491, you will see "Was this helpful?" and next to that "Provide feedback about the article."  The best place to provide a suggested edit is by clicking the "Provide feedback about the article." link and provide your information there.
