You mention Incident; is this a Jira Project or Jira Service Management?
If this is JSM, you could create different Request Types for the Portal, with different fields, etc - see: https://support.atlassian.com/jira-service-management-cloud/docs/what-are-request-types/
If this is Jira, to have different Screens each category would need to be an Issue Type, to allow you to have a different Screen per item - see: https://support.atlassian.com/jira-cloud-administration/docs/manage-issue-screens/
If you let us know if it's Jira or Jira Service Management, and any other specific details about what kind of customisations you would need per type, I'd be happy to try and provide some further ideas!

Thanks for your help this is what I was looking for.
