What is the best way to allow user inputs

I have a project where I need to add custom fields.
The fields are questions which has answers in yes/no.
I do not want to add them as custom fields as there are many in different tabs.

Is there any alternative method I can create such fields?

Also, I have few tabs like: Tab1, Tab2, Tab3. I want to hide some of the fields from Tab2 depending on the value selected from the fields in Tab1. Is there a way to achieve this?

Not out of the box. You'd have to use a plugin or scripting. FYI; tabs are just a logical separation of fields on the screen. Under the covers the fields are all on the same screen to JIRA.
