Why can't I find the "add" button in the Post Function to Transition?

When making a selection in the Post Function to Transition, I can't find the "Add" button (see screenshot).  I was able to do this awhile ago and now I can't figure out why it's missing.

This can happen when an add-on that provides post-functions is broken, but the first place to look is really the log file to see if there are errors in there when this screen is rendered.
