In a Jira project's Settings > Fields page I have removed all references to "Fix Version/s". It clearly says "No screens" for this field.
Yet the field keeps appears nonetheless.

It's a system field, built into the core and hence rather difficult to get rid of.
The best you can do is use "hide" in the field configurations - this will mostly get rid of it at a project level (it will still appear if people put data in it before you hid it)

That said, I think Atlassian should give much thought to how all of the working pieces come together to turn on/off fields. IMO there should be an Admin option on every single screen to hide/show individual fields, with a dialog box to control the scope of such a change.

Indeed. I've wanted to take "field configurations" out the back and put it out of my misery for years, moving the functions to screens instead.
