Why is my Jira Connection failing?

I'm attempting to set up my Jira Integration for qTest.

I've downloaded the Jira Integration app & tested the Jira connection successfully.
When I attempt to Save & Configure, it first says successful, and then changes to a message stating that there is an error configuring the connection & to ensure all my fields are correct.

I am not sure which of the fields are incorrect, so am at a loss as to how to move forward.

Hello, Good day. Could you check for any error's in HAR logs?

Also could you point to the location (URL) where you notice this error?
