XRAY: adding attachment to "Actual Result" does not work

When trying to add an attachment in the "Actual Result" dialog of the step of a test execution, we either get an error message or no message at all.

Is this a bug or are we doing something wrong?

Is this fixed? We still have that problem/issue.. :)
