This could be numerous issues.

For the users not being able to do transitions on the board - have you checked:

• The project(s) permission schemes - to ensure those users are in roles / groups that can transition issues?
• The project(s) user and roles - to ensure if permissions are based on roles, those users have access to the correct project roles to make transitions

^ I'd start with checking these two.

For the unexpected issues created and expected issues not being created, can you elaborate? What kinds of issues are being created or not created you are not expecting?

Automatic creation of issues could be an app (such as Automation for Jira), potentially through a workflow post-function or a script?
