I've a business requirements documentation form that need to be created in Jira, can anyone please help me on this.

Is should be created for a particular workflow or issue type?
If so create a transition screen.

Is a global form? That should be use for all users in the JIRA instance?
In that case I suggest you to create your own add-on to generate the report and save the data into the database.

Could you explain deeper what you need to do with this form?
