conditions and transitions in log action

Hi, dear all and dear  community admins,

Today I had new ticket from my PO, and the task consists of the sentence which is pasted below. 

"can you please add the conditions& transitions as stated in the "Log" actions starting with "If..." in the automation". 

I am pretty new in jira and this task is not clear to me.. The important part of this automation is also added to attachment.

It would be great, if you help me on this issue.
