form with cascading select

Hi, i want to make a form with cascading select like this,

I was trying but, i only can insert a cascading select in Jira Software (cloud), but i cant make a form in there (I suppose). I can make a form in Jira Work Management but I cant insert a cascading select.

Is there a way to make a form with a cascading select?

(It doesnt matter if it is made in Jira Software but I would like to make it on Jira Work Management)

(in the picture I suppose that isnt using external plugins or aplications)

@Johan Quinatoa Km You can add a cascading select in JSM.  If you are on a company managed project you will need to be a Jira admin to get it added.  If you are on a team managed project you will need to be the project admin to add it.
