Please go on jira.cprime.io and submit a bug report. This usually happens when you go from version 3.0 directly to 4.0, without the intermediate step). However, do not worry. Please submit your case on our Jira (jira.cprime.io), we'll help you migrate those scripts. Add logs, versions, etc

Can you please elaborate more on what the intermediate step is ? Its my 4th attempt to upgrade the instance now but the problem is still the same. The upgrade task failed to complete.
Any help would be appreciated.

The upgrade task did not complete, and this is why you have this error. Either Power Scripts failed to enable or something else happened there, so I will have to request you to open a support ticket on jira.cprime.io, the official support platform, we'll be happy to address that into it.
Now: please provide the atlassian-jira.log, and any other info you may think it helps (from version / target version, etc)
