Auto Unapprove for Bitbucket v4.0.0. not fully compatible with Bitbucket Server 5.15.1

This add-on (v4.0.0) seems not to work properly on Atlassian Bitbucket Server 5.1.5, pull requests are not unapproved automatically on changes. The config page on repo-level throws HTTP 500 error "Attempting to render undefined template 'aui.form.checkboxField'."

Downgrade to add-on v3.0.2 solves this issue.

Update to add-on v4.0.2 solves this issue.
