Capture for Jira

trying to use Capture for Jira

when I try to create an issue i get the following error:

The server returned the following errors:

• Field 'summary' cannot be set. It is not on the appropriate screen, or unknown.
• Field 'priority' cannot be set. It is not on the appropriate screen, or unknown.

checked on our project, and the above fields are defined as expected

any ideas?
