Some of the reporting packages for JIRA operate "outside of" JIRA's permission schemes and thus create additional administration overhead. Does Midori PDF respect JIRA's permission schemes? Does it also respect field level security?

For some reason, I was not notified about this question, but I found it accidentally after half a year. Better late than never. :-)
So, the JIRA PDF View Plugin fully respects JIRA's built-in security.
Nevertheless, field level security is not part of JIRA core! This is provided by a 3rd party plugin. These custom ACL rules are not obeyed by default. The good news is that because the PDF View Plugin supports customization both in the template and in Groovy scripts, these can be supported, too .
